Strengthening Your SNF Revenue Cycle:

Three Key Focus Areas

If you’ve been in long-term care for a while, you know how much the financial landscape has shifted. Managed care keeps growing, payer rules keep changing, and the old approach of submitting a claim and waiting to see what happens leaves too much cash on the table.

A healthy revenue cycle now takes a proactive, front-to-back strategy built on real-time information. There are three key areas to make sure your team gets right.

Focus Area 1: Verify Payer Eligibility Before SNF Admission

Your revenue cycle starts before the resident is admitted. You can’t afford to figure out coverage as you go.

Your team needs a complete, verified map of each resident’s coverage, including the true payers at risk, so everyone knows what to do. That includes traditional Medicare, Medicare Advantage/Part C, traditional Medicaid, managed Medicaid plans, and any subcontracted health plan arrangements within those plans (and those happen a lot). Missing just one layer of coverage exposes your facility to denials and write-offs.

Best practice is to use automated electronic eligibility verification directly from the payer rather than relying on phone calls, payer portals, or referral paperwork. Share the results with admissions, nursing, and billing so everyone works from the same information. Recheck twice a month since coverage can change mid-stay.

Technology can automate much of this process and help your team get the right coverage information upfront.

Focus Area 2: Optimize Payer Setups and Billing

Each payer has its own rulebook, and your UB-04 claims need to follow it. That means your payer setups in the EHR need to be correct, and your team needs a clear understanding of how each payer requires claims to be billed.

Sometimes that even means billing on paper. Whatever gets claims processed correctly and paid faster is the way to go.

There’s a straight line from admissions to nursing to billing to payment. To keep things on track, review your payer setups regularly and any time you see a pattern of denials from the same payer. A small setup error can affect every claim until someone catches it.

Focus Area 3: Track Claims and Performance in Real Time

You can’t manage what you can’t see. Many billing offices submit a claim and wait for the remittance or for business office staff to find time to research the claim. That can mean 45 days or more before you learn about a rejection, and then you may have to start from scratch.

Most payers now make claim status available electronically, so there’s no reason to wait. Check the status of claims within the first couple of days after submission and fix problems while there’s still time.

Your team should also have a dashboard that gives you a clear view of claim status and overall performance, including:

If your current system can’t give you that view, it’s worth finding one that can. The goal is to identify problems in days, not weeks or months.

Building a Stronger SNF Revenue Cycle

Managing an SNF revenue cycle in today’s environment requires everyone involved to have the right information at the right time. From verifying coverage before admission to getting payer setups right and monitoring claims after submission, small gaps can quickly turn into payment delays or write-offs.

The good news is that those gaps can often be identified earlier. A proactive approach gives your team more opportunities to address problems before they affect cash flow and helps create a more consistent path from admission to payment.
